In this practice, all the providers will have the opportunity to take part in the patient’s care. The MD/DO will see the patient and then the patient will see NP and PA in turn. This way, when one provider needs to take a vacation (as all humans will need to) the other providers already have a first hand experience of encountering the patient.

Strengths

Dementia especially within cultural contexts. We all have the same neurodegeneration but being more aware of the Asian aspects of filial piety and care taker burden is especially important to me.

Botox for spasticity under ultrasound. This is the logic challenge for understanding anatomy. There is such pleasure in seeing the spastic limb ‘open up' as well!

Weaknesses

Pain. Every clinical trial of any treatment for pain sets success at 50% reduction from original pain level. How many patients want that? Almost none. We are the standard of care so just keep that in mind. Standard is only 50% less than original.

Attention Deficit. In my mind, this is for the psychiatrist. I can work with the middle and high school levels to get services and cognitive behavioral therapy started within the school system but as an adult, the complications of attention deficit has now entered a much more psychologically complicated arena. If anything, we are good at partnering with whoever the psychiatrist the patient chooses to be.

Strengths

Stroke: Pre and post management. Identifying and evaluating risks for stroke is multifactorial. A proper evaluation of the brain circulation is very useful in establishing the level of risk as well as a talk point to start improving the overall well being of the patient. There is co morbid risk factors that many patients with the partnership of the primary care and specialists can improve. Hypertension, diabetes, smoking and alcohol are just four major points that can be affected.

Botox for migraine. Although there has been a bloom in recent treatment modalities for migraines with the development of the CGRP antagonists (‘gepants’ within the industry) and some of the even newer modalities involving electronic pulses and magnetic head sets, in the end, Botox has remained one of the few modalities that provide me the most satisfaction. How many injections can effect change so quickly and in such direct relation to the injector’s skill level? Headaches, like all other pain syndromes have been studied under the criteria that success is 50% reduction in pain level, but for migraine, many studies in all modalities have the achievement of zero level pain days. Very satisfying.

Weaknesses

Insomnia is better diagnosed through sleep medicine specialists. In the flushing area, we already have Dr. Kenneth Sha who runs a sleep lab. For a neurological sleep lab, the best sleep lab for this in New York is up in the Bronx at Montefiore Sleep Wake Disorders Center. We can do some basic sleep evaluation for obstructive sleep apnea which is the number one cause of morning headaches. But this is as an answer to the headache question and not a direct evaluation of insomnia.

Strengths

Brain Health is not just a function of your brain. This is the function of the body as a whole. Exercise is defined by many studies as bringing your heart rate to about 120 beats per minute for 20 minutes. Doing this at least once a week can bring valuable oxygen to the brain and help clear out dangerous metabolites and oxidants out of the brain. Nutrition is key. Not only should people focus on eating better daily but periodic pulses of various co-enzymes can aid in the liver in helping the system remain functional and balanced and thus the brain can work at the peak environment.

Botox for blepharospasm. The face is a person’s identity. When this identity is altered by developing facial spasms or twitches, it isn’t just about not being able to see from a closed eye or having saliva drool out, it can be about how you feel about your own self. There is not just a science to finding the correct muscles to inject to stop these abnormal movements but there is also an art where you try to bring the face to its most beautiful and self satisfying state: Your own face. What a pleasure to have a patient say that they look in the mirror and they see themselves again! Beauty of self confidence and self wellness with health are all connected.

Weaknesses

Chronic pain. Once pain becomes chronic, there is no longer this expectation that it should be completely extinguished. There are multiple factors that can cause pain that is intermittent to becoming a chronic issue. When this happens, patients should be educated that they have to deal with this on a daily basis by modifying lifestyle and changing habits as well as accepting sometimes there are limitations to their human bodies. It is still mind over matter in many cases. Chronic pain is best served in a multi specialty practice with a physical therapist, psychiatrist and a pain specialist all together on staff. One of the most successful Pain Centers is Hudson Wellness in Manhattan.
